Rare Disease Day Symposium 2020

One in 10 people in the United States has a rare disease. Come to the Rare Disease Day Symposium to connect with others in the West Michigan community who are interested in or have been impacted by rare diseases and disorders.

The Rare Disease Day Symposium aims to bring members of the rare disease community—patients, caregivers and families, researchers, advocates, and students—together to learn from one another and to support one another. The event is free and open to all who are interested in attending. The symposium will begin with a common morning session with invited talks from a scientist studying rare diseases, a medical geneticist and patients with various rare disease diagnoses. After lunch, participants will select breakout sessions to attend based upon their particular interests.

Waiver and Release

I understand that the Rare Disease Day (“Event”) may involve physical activity. On behalf of myself and any guests that I register, including my minor child(ren), on whose behalf I am acting, I accept participation in this event, and hereby waive and release Spectrum Health Hospitals and its affiliates, employees, agents, contractors, members, representatives, officers and directors (collectively, “Spectrum Health”)  from all claims or liabilities of any kind arising out of my participation and/or the participation of my guests/children in this Event.
 
I, on behalf of myself and my guests/children, authorize Spectrum Health an unlimited right  to take, use, re-use, create, re-create, modify, record, transmit, preserve, produce or reproduce, publish, exhibit, televise, display, or otherwise make available to others, images or likenesses of my or my guest/children’s face, body and/or voice, by any means available, including without limitation photograph, video, digital imagery, or other media not yet named or developed, for Spectrum Health’s use in any Spectrum Health-related publication, exhibit, newspaper article, Web site, poster, ad, television spot, billboard, or other communications format (“Authorization”). Spectrum Health’s rights with respect to this Authorization will continue under this Authorization is terminated by Spectrum Health or me.  I understand that I may refuse to agree to this Authorization.  I understand that I have the right to revoke this Authorization, if the revocation is in writing, except if Spectrum Health has taken action in reliance upon this Authorization.  I understand that in order to revoke this Authorization, I must submit a request in writing to:  Attn: Communications & Marketing, 100 Michigan Street NE, MC 57, Grand Rapids, MI 49503, or via e-mail at privacy@spectrumhealth.org
. I understand that if my personal image is used or disclosed pursuant to this Authorization that it may also be subject to re-use and re-disclosure by the party or media outlet receiving the information, and such use or disclosure will not be protected by the law.
 
By submitting this registration, I acknowledge that I have read, understand and agree to the above waiver and release and Authorization.
