Service Planning from Referral to Discharge 8.2020

An interactive training that introduces a team approach to service provisions by identifying needs, utilizing strengths, and building collaboration in order to promote change.

Learning Objectives:

  1. Participants will be able to eexplain the steps and processes from referral, intake, to assessment
  2. Participants will be able to describe the purpose of and describe the differences between psycho-social evaluation, family, and psychological assessment or interview
  3. Participants will be able to define strategies to identify strengths and recognize coping skills/styles and other resources
  4. Participants will be able to Learn to acknowledge an integrate cultural considerations
  5. Participants will be able to describe the structure for the initial service plan meeting and review
  6. Participants will be able to create a service plan document and use strategies to include the client in creating goals
  7. Participants will be able to complete a discharge summary and identify services, which will continue following discharge.

Registration fee is $40, which includes 3 Continuing Education Hours.
