This course will provide participants with an understanding of the role that clinical staff play in providing services for those experiencing chronic medical health conditions. Participants will review prevalence rates for chronic medical health conditions and the biopsychosocial impact of chronic conditions, as well as be introduced to chronic care models and a variety of intervention strategies.

The fee for this training is $45, which includes 3 continuing education hours.
