Root Cause Analysis (RCA)

Root Cause Analysis is a self-paced elearning course that outlines DoD Patient Safety Program's suggested practices for conducting RCAs. The course follows the RCA process from the initial reporting of a patient safety event through the formation of the RCA Team, identification of contributing factors and root causes, and the recommendation of corrective actions. It is designed to provide all levels of personnel in military treatment facilities with the information they need to participate effectively on an RCA Team, and it provides additional material for Patient Safety Managers and RCA Team Facilitators.

The course takes approximately 90 minutes to complete. All learners who complete the entire course, including the end-of-course posttest with a passing score of 80% or better and the evaluation, will receive a Certificate of Completion.  

To participate in this FREE online activity, you will need the following type of hardware and software: Computer with Internet connection and Microsoft Internet Explorer version 8 or above required to run course. Course is compliant with Section 508 of the Americans with Disabilities Act (ADA) and includes text captioning for audio components.
